ILLNESS/ABSENCES: Please contact the school office in the morning if your child is to be absent

I have been working on updating your child’s medical information in our electronic system,
CareDox. If you haven’t already done so please access CareDox to complete your child’s health
information. This will keep your child’s health file up to date. Whenever your child has an
updated physical and/or further immunizations please send a copy into the health office for me
to review and keep on file. This is very important in order to care for your child accordingly, if
there have been any changes in his/her medical histories.
If I can be of any assistance to you throughout the year please don’t hesitate to call or e‐mail
me.
